The service index is a list of definitions used to describe the services provided by the agencies listed in the
Adherence/Compliance with HIV Medications: Counseling or special programs to ensure readiness for and adherence to HIV/AIDS treatments.
Case Management: Professionals who link clients with primary medical care and other services to ensure coordinated access to
health care and support services.
Counseling-Personal and Family: Counseling for personal problems, coping with loss or tragedy, or relieving family crisis.
Education: Specialized instruction regarding HIV prevention or HIV care and treatment.
Faith Initiative Project: Initiative helping faith communities develop positive responses to the HIV epidemic.
Financial Assistance: Provision of short-term payments for transportation, food, essential utilities or medication assistance.
Food Bank: Provision of food, meals or nutritional supplements.
Housing and Shelter: Provision of short-term or emergency financial assistance to support temporary and/or transitional housing to
enable an individual to gain or maintain medical care.
Legal Assistance: Provision of legal services directly necessitated by a person's HIV status.
Medical Treatment: Professional diagnostic and medical services from a physician, physicians' assistant, clinical nurse specialist or
nurse specialist or nurse practitioner in an outpatient, community-based or office-based setting.
Mental Health: Psychological and psychiatric treatment and counseling services.
Nutrition: Provision of nutritional education and/or counseling provided by a registered dietitian.
Outreach: Street/Community: Identifying individuals in a community at risk or who may not known their status so they can be HIV
Substance Abuse Treatment: Provision of treatment and/or counseling to address substance abuse issues provided in an outpatient or
residential health service setting.
Support Group: Periodic group meetings where individuals in similar circumstances get together to provide support to each other.
Syringe Exchange Program:
Translation: Language translation or translation for the hearing impaired.
Transportation: Transportation services provided to a client in order to access primary medical care or psychological support
